Does the RealWorld Feedback live up to the “hype” – claimed benefits?
Updated 4–8- 25
- Yes!! in regards to Energy, Mitochondrial health, Neurorestoration –
Repair NAD+ is easily in the TOP 5 all time discussion. - NAD+ is easily in Top 5 for best reviewed compound in general.
It has life changing potential which puts it into a truly unique class of tools! - I will absolutely give it the Project Biohacked Stamp of Approval
- This has personally been a tool that has changed my life and helped in so many different ways.
- It’s not a one size fits all PANACEA compound but it performs extremely well for a very high percentage of people.
Mechanism of Action: How Does NAD+ Work?
WHAT IS NAD+?
Nicotinamide Adenine Dinucleotide (NAD⁺) is a pyridine nucleotide coenzyme present in all living cells. It serves as a:
- Coenzyme in redox reactions (NAD⁺ ↔ NADH)
- Substrate for signaling enzymes (e.g., sirtuins, PARPs, CD38)
- Metabolic regulator, linking nutrient availability to cellular function
DUAL ROLES: METABOLIC COENZYME + SIGNALING MOLECULE
1. Redox Reactions (Energy Metabolism)
Main role: Electron carrier in catabolic pathways
Key Pathways:
- Glycolysis: NAD⁺ accepts electrons → NADH
- TCA Cycle: NAD⁺ reduced to NADH in multiple steps
- Oxidative Phosphorylation: NADH donates electrons → ATP production
in mitochondria
Why it matters: High NAD⁺ levels = more efficient ATP production and
mitochondrial function
2. Sirtuin Activation (Epigenetic Regulation & Stress Response)
Sirtuins (SIRT1–7) are NAD⁺-dependent deacetylases that modulate:
- Gene expression (via histone deacetylation)
- Mitochondrial biogenesis (via PGC-1α)
- Inflammation (via NF-κB suppression)
- Autophagy and apoptosis
SIRT1/SIRT3 Activation by NAD⁺ =
→ Increased mitochondrial resilience
→ Reduced oxidative stress
→ Enhanced neuronal survival and synaptic plasticity
3. PARPs (DNA Repair Enzymes)
Poly(ADP-ribose) polymerases (PARPs) detect DNA damage and use NAD⁺ as a
substrate to:
- Add ADP-ribose polymers to DNA repair proteins
- Facilitate DNA strand break repair
High DNA damage → Increased PARP activity → NAD⁺ depletion
Chronic activation (e.g., in aging or neuroinflammation) = NAD⁺ drain → mitochondrial dysfunction
4. CD38/CD157 (NAD⁺-Consuming Enzymes)
These enzymes degrade NAD⁺ and are involved in:
- Calcium signaling (via cyclic ADP-ribose)
- Immune cell activation
- Age-related NAD⁺ decline
In aging and neuroinflammation, CD38 overactivation contributes to NAD⁺
depletion
5. Neurotransmitter Synthesis & Cognitive Function
NAD⁺ is a precursor in reactions involved in:
- Serotonin synthesis (via tryptophan metabolism)
- Dopamine balance and receptor sensitivity
- GABA/Glutamate homeostasis
Also indirectly supports: - BDNF expression via SIRT1 activation
- Neuronal energy metabolism and myelin maintenance
NAD⁺ DEPLETION & DYSFUNCTION
Causes of NAD⁺ decline:
- Aging
- Chronic inflammation
- Substance abuse
- Overactive PARPs/CD38
- Metabolic stress (e.g., insulin resistance)
Consequences:
- Mitochondrial dysfunction
- DNA damage accumulation
- Inflammatory cytokine surge
- Impaired neuroplasticity and cognitive decline
- Cellular senescence
NAD+ BIOSYNTHESIS PATHWAYS
1. Preiss–Handler Pathway (from nicotinic acid)
2. Salvage Pathway (from nicotinamide or NMN)
- Most dominant in mammalian cells
- Rate-limiting enzyme: NAMPT
3. De Novo Pathway (from tryptophan)
NAD⁺ precursors like NR, NMN, and NA feed into these pathways
NAMPT activity is crucial — stimulated by fasting, exercise, and certain
polyphenols (e.g., resveratrol)

RESEARCH DOSING:
(Educational Purpose ONLY – Project Biohacked LLC)
(Educational Purpose ONLY – Project Biohacked LLC – Always consult your physician before making any changes to your medical treatment plan)
Dosing for NAD+ (subcutaneous injectable form) is a rabbit hole topic, there’s a WIDE RANGE of dosages and protocols.
Quick Dosage Range Explanation:
- There is NO one size fits all dosing approach!! No cookie cutter protocols despite many “experts” handing them out.
- The approach I have taught to clients over last 7 years is a very simple
one. - We start low – see how Individual responds THEN taper up based on their response.
- We look for the dosage where they notice the most benefits without any side effects.
- What dose and frequency works best per person will vary!!
- For clarification the “Microdose – Test Dose” is for those who tend to be VERY SENSITVE to exogenous compounds. People who may have a chronic health condition like MOLD, CIRs, Lyme, Autoimmune, MCAS.
- The more chemical sensitive someone is the lower they should test dose. Simple principle that ive found to work great for people 🙂
Dosage Range for NAD+ Subcutaneous INJ:
“Microdose” – Test Dose Range: 10mg – 25mg
Low – Moderate Dose Range: 25mg – 50mg
Moderate Dose: 50mg – 75mg
Moderate – Moderate High: 75mg – 100mg
High: 100mg – 125mg
Upper End: 125mg – 150mg+
How to QUICKLY Find the right dose at which you will notice benefits from NAD+:
Example of my personal approach – approach with clients:
- Someone has never tried NAD+ injectable form before.
- They decide to go with a lower test dose of 25mg.
- They notice NO side effects and NO benefits.
- The next day I would have individual trial a higher dose – likely 50mg
- Again if they notice no side effects and no benefits I would have them
increase. - We will repeat this process until we either notice benefits or run into the NORMAL side effects from NAD+
- Some people need to go as high as 100 – 125mg to FEEL the energy
kick!! - Some people unfortunately don’t feel good from NAD+…. this is rare
and we don’t full understand why but if you experience ZERO benefits even at 100mg you may just be a non responder sadly. - But the vast majority notice benefits and if it’s a good fit then it’s an amazing tool for a fast NATURAL non stimulating boost in energy!!
Setting Expectations and a note on NON RESPONDERS:
- Some people will notice a energy boost day 1 and at doses as low as
10-25mg - Majority of people find their sweet spot between 50mg – 100mg
- Some people will need to do a “loading protocol” to notice benefits.
- IE – run NAD+ for 2 weeks at 3-4x per and at the highest dose they
tolerate side effects at. I prefer that this is in the 25mg – 100mg range
but for some they MAY need to go higher if they notice nothing at all from 100mg. - NON Responders – unfortunately there is a small percentage of people who do not do well with NAD+ and the possible reasons why are vast. Could be genetics, could be current toxin load, chemical sensitivity from immune based conditions. I wish I had the answer but it is complex.
Dose RANGE that works best for me after 4+ Years Cycling:
- General Fatigue Needing Quick Energy Boost 50 – 75mg
- ADD/ADHD Burnout – Heavy Fatigue – Poor Night Sleep 100 – 125mg
- Sleep deprived – Extreme Fatigue 150+mg
Couple Quick Notes on my personal usage:
- I have personally gone as high as 150mg – 200mg+ in a day w/ NAD+ but I very slowly tapered up to that higher dose.
- Typically 100mg – 150mg is plenty to give me a much needed energy boost. Those higher doses I use on days I am very sleep deprived and or going through a very high stress period.
- Per Dr. Matt Cook he spoke briefly about doing 200mg 1x per week for addiction recovery patients. Personally I would think 100mg 2-3x per week as needed would be more ideal but just my opinion based on my experience.
Frequency:
- My personal approach is 2-3x per week on a as needed basis and the few protocols from the professional community also seem to like this approach 🙂
- I turn to NAD+ on those days I really need some help with energy.
- Specially since it works extremely quick for myself it’s a tool I try to save for the times I really need it. Ive found this approach to keep it at
maximum efficacy for myself.
Cycle Length:
- I like 8 – 12 weeks w/ 6 – 8 weeks “off” for just general health and or
managing a higher stress period in life. This 8 – 12 week cycle also aligns with the few professional recommendations I have come across. - There is NO set in stone approach on this part, it is highly individual
based on goals and health situation.
Some individuals who struggle with chronic fatigue do opt to run longer term as they continue to try and address root cause of fatigue.
ChatGPT Protocols Overview:
– I find the following information to be generally pretty accurate and worth considering!
NAD+ Injectable Dosing Protocol
The dosing protocol for NAD+ injectable form depends on your goals (e.g., energy

Current Review of Potential CANCER Risks:
NAD+ and Cancer Cell Metabolism:
Cancer cells often exhibit altered metabolism to support rapid proliferation. NAD+ is essential for glycolysis and other metabolic pathways that are upregulated in cancer cells. Elevated NAD+ levels can enhance these metabolic processes, potentially facilitating tumor growth. Consequently, targeting NAD+ metabolism has been proposed as a therapeutic strategy to disrupt cancer cell energy production and inhibit tumor progression. Scientific American+1PubMed Central+1
NAD+ in Cancer Therapy:
While NAD+ is vital for normal cellular functions, its role in cancer therapy is dualfaceted. Inhibiting NAD+ synthesis has shown promise in reducing tumor growth, as cancer cells rely heavily on NAD+ for their metabolic needs.Conversely, enhancing NAD+ levels may bolster anti-tumor immunity by improving the function of immune cells. This dual role suggests that therapeutic strategies should be carefully tailored to the specific cancer context. Nature+1jinfiniti.com+1Frontiers
Considerations for NAD+ Supplementation:
Given the complex relationship between NAD+ and cancer, individuals with a history of cancer or those at high risk should consult healthcare professionals before initiating NAD+ supplementation. The potential for NAD+ to support both normal and cancerous cell functions necessitates a personalized approach to its
use. PubMed Central
Conclusion:
NAD+ plays a multifaceted role in cancer biology, acting as both a supporter of essential cellular functions and a potential facilitator of tumor growth, depending on the context. Ongoing research is crucial to fully elucidate these relationships and to develop safe and effective therapeutic strategies involving NAD+
modulation.
